A RESOLUTION AUTHORIZING THE SUBMISSION OF A GRANT APPLICATION TO THE CENTERS FOR DISEASE CONTROL AND PREVENTION (CDC) FOR PUBLIC PREVENTION HEALTH FUND: COMMUNITY TRANSFORMATION GRANTS

WHEREAS, the Cherokee Nation since time immemorial has exercised the sovereign rights of self-government in behalf of the Cherokee people;
 
WHEREAS, the Cherokee Nation is a federally recognized Indian Nation with a historic and continual government to government relationship with the United States of America;
 
WHEREAS, The purpose of this program is to create healthier Cherokee communities by building capacity to implement broad evidence and practice-based policy, environmental, programmatic and infrastructure changes to prevent heart attack, strokes, cancer and other leading causes of death or disability;
 
WHEREAS, Community Transformation Grants will achieve these outcomes via a variety of projects with schools, businesses, faith-based organizations, community organizations, and health care providers;
 
WHEREAS, The focus of the project will be to serve as a catalyst for environmental, policy and infrastructure change in communities by specifically addressing five "Strategic Directions:" tobacco-free living, active living and healthy eating, high impact evidence-based clinical and other preventive services, social and emotional wellness, and healthy and safe physical environment.
 
BE IT RESOLVED BY THE CHEROKEE NATION, that the Principal Chief and the Council of the Cherokee Nation authorized that a proposal be submitted to the Centers for Disease Control and Prevention to provide service to enable Cherokee communities to reduce the burden of chronic disease in the Tribal Jurisdictional Service Area toward the ultimate goal ensuring a healthy and productive Cherokee Nation.
